Registration Procedure
The registration form can be downloaded here. Please register by one of the following methods:

1. via internet at www.hkicc.org
2. by fax at (852) 2816 5258
3. by mail

Please send the completed registration form and a crossed cheque payable to "The University of Hong Kong" with the appropriate fee to the Congress Secretariat.

Local (HK) Medical Doctor	Daily Rate : HK$400 Half Day on 3 Nov : HK$200
Local (HK) Nurse / Allied Health / Non-medical Professional / Postgraduate Student	Daily Rate : HK$150 Half Day on 3 Nov : HK$100

Payment Method
All payments are to be made in US or HK Dollars by either bank draft (US$) or Cheque (HK$) payable to "The University of Hong Kong". Payment should be sent with the completed registration form to the Congress Secretariat. On-site registration may be paid in cash, cheque or traveller's cheques.

Registration and Schedule
Registration will begin on Wednesday, 3 November, 2011 at Reception, G/F, William MW Mong Block, Li Ka Shing Faculty of Medicine. Please present the confirmation letter at the Reception to obtain your registration package. The schedule is as follows:

3 November, 2011, Thursday	1:00 pm - 4:30 pm
4 November, 2011, Friday	8:30 am - 4:30 pm
5 November, 2011, Saturday	9:00 am - 4:30 pm

Official Receipt
An official receipt will be issued upon payment of the registration fee.

Entitlement
Full registration delegates will be entitled to:

participate in all scientific sessions
visit the Exhibition
sandwich lunch
coffee and tea during the morning and afternoon breaks
receive a set of official publications
attend the Opening Ceremony

Day registration delegates are entitled to:

participate in scientific sessions on the day
visit the Exhibition
sandwich lunch
coffee and tea during morning and afternoon breaks
receive a copy of the Final Program

Cancellation of Registration
Provided notification of cancellation is received on or before 3 October, 2011, 50% of the fee will be refunded. No refund will be made for cancellations received after 3 October, 2011.
